Intake Paperwork

Please print and fill out all necessary intake paperwork before your first scheduled session. You will also need to print out and read through the Practice Policies as well as the HIPAA policies form. If you are unable to print out a hard copy, let me know and I will have a copy ready for you to fill out fifteen minutes before your first appointment.
